SUD diagnostic assessments Treatment planning Individual and group facilitation Discharge planning Documentation of services in the electronic clinical chart Implementation of evidence-based practices for substance use disorders Linking patients to the recovery community and available resources Collaboration with multidisciplinary teams Mission Statement:

Qualifications include:
Must have current LCDCII (Licensed Chemical Dependency Counselor II) LCDCIII (Licensed Chemical Dependency Counselor III) LPC (Licensed Professional Counselor) LSW (Licensed Social Worker) licensure or higher.
